Professional contact information only

Hospital, facility and listing-correction forms ask for your name, professional role, organization or program, work email, phone where requested, and business information about pilot interest or a listing. Optional notes must describe your organization or public listing only. Do not enter patient information or upload files; this site provides no patient-upload service.

How we use and store it

We use submissions to answer enquiries, coordinate a free pilot, and review directory additions or corrections. Submitted professional information is stored in the site's Cloudflare database and is available only to authorized maintainers. After the site saves your enquiry, your browser sends its professional contact fields through FormSubmit to thaole0914@gmail.com for follow-up. Your browser reports the forwarding result back to the site; that report is not independent proof of inbox delivery. FormSubmit and the destination email provider process the message to provide those services. A forwarding acknowledgement does not establish inbox delivery.

See FormSubmit's privacy terms. No patient referral is created by submitting a form. We do not use form submissions to sell personal information or enroll you in marketing lists.

Browsing and authentication

Search and compare selections are held in page memory. The fictional portal resets when refreshed. The hosting provider may process technical request data such as IP addresses and browser information for operation and security. Public forms do not require sign-in. The direct maintenance area uses ChatGPT authentication and an owner-managed access list. No additional analytics service has been installed in this phase.

Retention, corrections and deletion

Contact us to ask about, correct, or request removal of your professional submission. The owner must finalize retention and deletion procedures before launch; automated retention expiry is not implemented. Copies forwarded to email require separate mailbox handling. FormSubmit's help page states that unactivated submissions may be retained for 30 days.

Questions or accidental patient information

Contact thaole0914@gmail.com. If you submitted patient information by mistake, tell us which submission to remove without repeating the patient details. Do not use this site for care decisions, emergencies or private clinical communication.
